Enter a radiation therapy treatment
Submit radiation therapy treatment plans for preauthorization.

Enter the treatment dates

After you verify the diagnosis details and answer any follow-up questions, we ask you for the treatment dates.

Note: If the treatment plan needs medical review, you'll see the Medical Review Required screen before you enter the treatment dates.

On the Enter Treatment Plan Dates screen, enter the treatment plan start date.

The treatment end date automatically calculates based on the maximum cycle information. We add a three-week cushion to account for any delays that may occur during treatment.
Update the dates if necessary, and then click Save and Continue.

If the treatment start or end date changes after you submit the treatment plan, call the payer to update the treatment dates on the authorization. You can't update the treatment dates in Eviti Connect.
